Payment Policies

Payment is due day services are rendered. Your insurance company requires you pay your co-payment at the time of service. Coinsurance is also due at time of service. A finance charge of 1.5% per month will be assessed for payments received after 30 plus days from date of service.

I accept check, cash, and am a preferred provider for: Premera Blue Cross, Regence, First Choice and Aetna. There will be a $25.00 fee applied to your account for any returned checks.
